Securities Held Electronically

If your securities are held "in street name" with your broker, download this Stock Transfer Form and send a completed copy to your broker to transfer your shares to UCDS and to UCDS we know who to thank. It is wise to check with your broker and with UCDS soon after sending this form to ensure that the transfer is complete.

Note:

  • If you are transferring stock from a joint account, both account owners must sign the form.
  • You can contribute stock from several companies held within the same account.
