  • Personal reference is someone who can speak to your interest in education and your strengths.

You may choose 2 professional/academic references in place of a personal reference.

Reference Waiver Form

The Family Educational Rights and Privacy Act (FERPA) provides individuals with, among other things, the right to review their education records, which may include confidential letters (“Letters”) of recommendation. However, you may waive your right to review Letters written on your behalf that relate to any of your applications for admission and employment at UCDS. A waiver of this sort is voluntary. Waiving your right lets recommenders know that you do not intend to read your recommendations, which helps reassure institutions that letters are candid and truthful. Some recommenders may decline to write a letter for you if you do not waive your rights.

Indicate whether you wish to waive your right under FERPA to review all of these Letters by making your selection below.

After you make your selection and submit this form, you will receive an email within 3 days with a link to the appropriate recommendation form to send to your references. Please reach out to gse@ucds.org if you do not receive the email.
